Role Description The Consultant Pharmacist is a full-time, on-site role based in Fort Collins, CO, supporting long-term care facilities with expert medication management and regulatory compliance. This role conducts comprehensive medication regimen reviews, evaluates pharmacy programs, and works to reduce medication errors while promoting safe, effective prescribing practices. The Consultant Pharmacist collaborates with interdisciplinary care teams, participates in clinical rounds, and provides education to nursing and administrative staff on medication-related policies, procedures, and best practices. Daily responsibilities include documenting findings and recommendations, assisting with formulary review, supporting survey readiness, and contributing to corrective action plans when needed. The role also involves analyzing workflow and medication-use processes, offering practical improvement strategies aligned with resident-centered care and regulatory standards. Qualifications Active Pharmacist license in Colorado (or eligibility for licensure) and Doctor of Pharmacy (PharmD) or equivalent degree. Experience in long-term care, skilled nursing, assisted living, or geriatric pharmacy practice, with knowledge of federal and state regulations affecting medication use. Strong skills in medication regimen review, medication safety, formulary management, and clinical decision-making in older adult populations. Ability to interpret and apply regulatory guidelines, support survey readiness, and contribute to compliance-focused policies and procedures. Proficiency in collaborating with interdisciplinary teams, communicating clinical recommendations clearly, and educating clinical and non-clinical staff. Comfort with electronic health records, pharmacy systems, and basic data analysis to identify trends and improvement opportunities. Demonstrated ability to work independently on-site, manage multiple facility engagements, and prioritize tasks in a dynamic environment.
